# FSSAI Quarterly Self-Audit Checklist

FSSAI's audit logic is unforgiving. Compliance scores 2 points (4 for critical items). Grades run from A+ at 90% or above, down to B at 50–79%. A single critical failure caps the achievable grade, regardless of everything else. The quarterly self-audit runs your own premises against that logic first. It also covers the deep-clean, pest-proofing and governance items on a quarterly rhythm.

The checklist · 10 checks · every check cites its legal basis

### Scored self-audit

**1\. Conduct a full self-audit of the outlet against the complete item bank using FSSAI-style scoring (2 points per item, 4 for critical; any critical failure caps the grade); record score and grade.** Required under FSSAI third-party audit methodology and Hygiene Rating scheme. Enforcement precedent: Establishments graded below threshold face enforcement priority.

**2\. Run or verify readiness for a mock recall: trace one ingredient lot from receipt to sale within 4 hours; document gaps. Execute a full drill at least annually.** Required under FSS Act 2006; FSSAI audit expectations. Enforcement precedent: Recall readiness a standard audit line.

**3\. Review third-party audit status: audit scheduled/completed per FSSAI applicability for the business category; findings closed.** Required under FSSAI third-party audit framework (AMS). **4\. Hold a management review: trend the quarter's daily/weekly/monthly findings, identify top three recurring failures, assign an improvement plan with owners and dates.** Recommended practice - helps continuously improve your food safety management system (FSMS). **5\. Verify emergency documentation set is current: incident-reporting contacts (FSO, local FDA office), crisis SOP, product-liability insurance where held.** Recommended practice - Preparedness practice. Enforcement precedent: Adjudication proceedings in Bengaluru show incident-response burden.

### Governance & follow-through

**6\. Confirm the FoSCoS annual return is filed by the 31 May deadline (verify each quarter; file in Q1 of financial year); acknowledgment archived.** Required under FSS (Licensing & Registration) Regulations – annual return. Enforcement precedent: Return non-filing attracts penalties and inspection priority.

**7\. Review the training calendar: refreshers delivered, new supervisors certified, training records complete for every current employee.** Required under FoSTaC requirements; Schedule IV training. Enforcement precedent: Training records checked in full audits.

### Deep clean, pest-proofing & water

**8\. Complete the quarterly deep-clean: hoods, ducting, cold-room strip-down, dry-store empty-and-clean, behind fixed equipment; certificate/photos on file.** Recommended practice - Supports Schedule IV maintenance obligations. Enforcement precedent: Grease/duct build-up part of infrastructure findings.

**9\. Structural pest-proofing review: door sweeps, window screens, drain covers, wall penetrations sealed; proofing gaps closed within the quarter.** Required under Schedule IV – premises pest-proofing. Enforcement precedent: Infestations recur where proofing, not just treatment, fails.

**10\. Verify water-tank cleaning completed per semi-annual schedule with records and post-clean test.** Required under Schedule IV – water storage hygiene. Enforcement precedent: Water storage checks standard in inspections.

**Failing the scored audit - one critical failure caps the grade**

**Missing food-handler medical records at inspection**

Absent handler medical records were among the stated grounds at MIG Cricket Club in the July 2026 Mumbai suspensions. Records are checked at inspection as compliance evidence. If it isn't documented, it didn't happen.

**Pest infestation and unclean premises found at inspection**

Six FSSAI licences were suspended in four days in July 2026. The grounds are attributed per establishment. At the Cricket Club of India: live cockroaches and flies, expired food and mouldy vegetables. At MIG Cricket Club: cockroach infestation and unhygienic conditions. At Willingdon Sports Club: insanitary floors and dirty drains. The sixth, Poornima Restaurant, has petitioned the Bombay High Court. It is challenging its suspension and disputing the findings. So no finding is treated as established against it.

**Clogged drains and stagnant water on premises**

Cited repeatedly in the Mumbai suspension orders. Stagnant water is treated as a pest-breeding and contamination ground. It is also one of the first things a walkthrough inspection sees.

A single critical failure. It caps your grade no matter how strong the rest of the audit is. In 2026 enforcement, the recurring critical territory was clear.
